Overview

Are you a passionate and skilled Registered Nurse (full time, nights) seeking a rewarding hybrid opportunity in patient care? As a crucial member of our dynamic team, you will play a pivotal role in our Hospital at Home team. Responsibilities include assisting with disease management, medication management, education, care planning and more for patients who have left the hospital and are now being managed at home.

Position Highlights

  • Shift/Schedule: Full time nights (7pm-7am),three 12-hour shifts per week, opportunities available, with weekend rotation
  • Department: Virtual Care
  • Location: Hybrid position

What You’ll Need:

  • Graduation from an accredited School of Nursing required.
  • BSN preferred
  • At least 5 years of bedside nursing experience. Will consider 4 years if ED/ICU nursing experience
  • Holds an active license as a Registered Nurse in the state(s) in which the client(s) are located or holds an active multistate license in any Nurse Licensure Compact member state. If the client(s) location is not in a NLC member state, the nurse must apply for a RN license in that state and should defer from practicing in that state until a license has been obtained. The licensed nurse is responsible and accountable for knowing, understanding, and practicing in compliance with the laws, rules and standards of practice of the state(s) where the client(s) is/are located.
  • Basic Life Support for Healthcare Provider (BLS HCP) from AHA required. Advancement complies with the requirements of the Professional Nurse Clinical Advancement Program. Experience, approved certification, and nursing education as outlined in Professional Nurse Clinical Advancement Program
  • Adaptability 
  • Comfortable managing technology 
  • Microsoft tool experience
  • Strong critical thinking and communication skills
  • Ability to work independently

  • Demonstrate knowledge of the principles of growth and development  
  • Demonstrate the skills and competency appropriate to the ages, culture, developmental stages, and special needs of the patient population served.

Job responsibilities include, but are not limited to:

  • Assisting with disease management, medication management, education and care planning for patients who have left the hospital and are now being managed at home. 
  • Assessing and directing comprehensive nursing care using telephone triage protocols. 
  • Providing and assessing effectiveness of patient family education initiatives.
  • Performing telephone triage with diverse patient population.
  • Assessing needed care and collaborates with patients, and others, to provide clinical solutions.
  • Providing comprehensive nursing assessment in an untraditional nursing environment.
  • Actively participating in team conferences and helps assess patient education needs.
  • Notifying each practice, physician, and/or managed care client after each encounter with patient, parents or managed care clients regardless of follow-up needs.
  • Notifying designated leadership of operational concerns.

Physical Requirements

  • Long periods of sitting and utilizing a telephone headset.
  • Walking, standing, lifting, reaching, stooping, bending, pushing and pulling. 
  • Must be able to lift 25 pounds. 
  • Must speak English and adapt to fit the patient audience while conveying information. 
  • Intact sense of sight, hearing, smell, and touch. 
  • Good finger dexterity. 
  • Critical thinking skills and ability to concentrate in a fast-paced environment with numerous interruptions. 
  • Must be able to respond quickly to changes in patient or call center conditions.
